INSTRUCTIONS:  These questions only apply to people paying phone rates to communicate with county or state prisoners and do not apply to federal prisoners.  It also does not include any states where prepaid phone cards are allowed.  

 

If you have more than one prisoner you communicate with in different states, please fill out a separate questionnaire for each one.

 

In completing this questionnair by email, you are attesting that these statements are true to the best of your knowledge.

 

  A.  What is the name of the company charging your phone bill?  Please type exactly as the name appears on your bill.

 

 

 

  B.  If more than one company has charged you in the last 4 years type in the names of the others companies.

 

 

 

  C.  Do you have alternative ways of receiving prison phone calls or is this method the only one available?

 

 

 

  D.  Are you charged a surcharge for connecting the call?  If so, what is the surcharge amount?

 

 

 

  E.  Average per-minute rate for each phone call received (call charge minus surcharge divided by call length).

 

 

 

  F.  Is there a time limit on the phone calls?  If so, what is the time limit?

 

 

 

  G.  Are recorded warnings broadcast during the call when either you or the caller cannot hear each other's conversation during the broadcast?

 

 

 

  H.  What is your current standard dial out rate for long distance from your home phone (per minute charge)?

   L.  Estimate of the total amount of money paid for prison phone calls over the past 4 years.

 

 

 

  M.  Do you experience call drops, call blocks, poor call quality, etc. (these issues would be things that do not occur with your usual non-prison phone calls)?  If so, please list these issues briefly. What we are looking for is a comparison between technical the quality of standard calls vs. technical quality of prison phone calls.
